The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves. 

The Colorado Coalition for the Homeless seeks a Director of Nursing who is responsible for the continuous improvement of nursing care, health education, and preventive services throughout CCH, in collaboration with other Integrated Health Services disciplines.   This includes sites such as    Stout Street Health Center (SSHC), Samaritan House Clinic, CCH Respite Programs, West End Health Center (WEHC), Fort Lyon Health Center (FLHC), St. Francis Center, the Health Outreach Program (HOP), and all Supportive Housing and Program Services.  The DON ensures that adherence to current federal, state, and local standards, and regulations that govern CCH, in order to ensure that the highest degree of quality care is maintained and standardized at all times.  This position is located at Stout Street Health Center, 2130 Stout Street, Denver, CO  80205 and reports to the Chief Operating Officer.

Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
 

  1. Leads strategic planning for Nursing Services, in collaboration with other Integrated Health Services leaders and departments.  Uses innovative approaches to build an engaged and creative RN and MA workforce that is prepared to meet ongoing needs of growing homeless population.
  2. Leads and equips RN and MA team to navigate changing healthcare delivery models by creating infrastructure to meet quality outcomes.  This includes overseeing the development, implementation, and maintenance of a nursing-led population health infrastructure, nursing-led transitional care management infrastructure, and risk stratification infrastructure.
  3. Oversees, coaches, and ensures ongoing quality assurance and quality improvement efforts.
  4.  Is responsible for policies and procedures, onboarding procedures, ongoing training needs, credentialing and privileging, competencies, in-services, and peer review to ensure a well-educated department.
  5. Champions Patient Centered Medical Home philosophy.
  6. Continuously improves the integration of medical and psychiatric/behavioral healthcare and housing through RN and MA services and their standardization.
  7. Acts as liaison between Nursing Services and the community at large.
  8. Oversees the development and implementation of evidence-based staffing ratios that utilize every staff member to the highest degree of licensure and scope of practice.
  9. In collaboration with the Integrated Health Leadership team, participates in the development, implementation, and maintenance of a telemedicine program at CCH in relation to nursing services.
  10. Leads the development, implementation, and maintenance of a high-quality teaching facility for MA, nursing, and other student rotations. 
  11. Oversees budgets and grant projects related to Nursing services.
  12. Ensures the competency of all Nursing staff members and that those staff are practicing within their scope of practice, in accordance with the laws and regulations of the State of Colorado.
  13. Provides and encourages professional development and coaching and participation in Integrated Health Care leadership, with emphasis on  trauma-informed supervisory principles.    
  14. Provides direct services to patients/clients at various CCH sites (at least 4 hours per week) in order to keep abreast of changing patient/client needs and maintains EHR core competencies.   
  15. Reviews Client and Staff complaints that are germane to Nursing Services and investigates and takes appropriate corrective actions as needed.
  16. Performs other duties as assigned.

Qualifications Summary
 

  1. Registered Nurse currently licensed in the State of Colorado.
  2. Previous experience as a supervisor in a community health center, hospital, long-term care facility, or other related health care facility.
  3. Previous experience as an RN, preferably in ambulatory care, public, or community health environment.
  4. Certification in Basic Life Support (CPR).
  5. Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to skilled nursing care.
