LPN Residential Care Coordinator

Licensed Practical Nurse/LPN, Part Time, Day Shift. 0.5 FTE

Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Licensed Practical Nurse/LPN provides nursing care and services for our residents.

Licensed Practical Nurse/LPN Job Responsibilities:  

• Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday/weekend operations.  
• Serves as coordinator of care for scheduled weekend and holiday shifts.  
• Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin. 

Licensed Practical Nurse/LPN Job Requirements:  

• Education: Diploma from accredited program in practical nursing required. 
• Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing 
• Certification: Current CPR/AED certification
