_{Community Care is hiring a Personal Care Workers/PCWs for the Day Shift at our Assisi site in St. Francis, WI. _{Personal Care Worker/PCW Job Responsibilities:}  

_{• Provide various care services necessary for attending to the personal needs}

_{• Support the health of Community Care program members}

_{• Serve as coordinator of care for scheduled weekend and holiday shifts}  

_{• Ensure care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care}  

_{Personal Care Worker/PCW Job Requirements:}  

_{• Education:  High School diploma or GED preferred} 

_{• Experience: Six months of experience providing personal care services for elderly or disabled adults in a group home, assisted living or long-term care environment and experience working with individuals with dementia.} 

_{Personal Care Worker/PCW Job Schedule:} 

_{• Hours of Operation, 6:00 AM – 2:30 PM (20 Hours Per Week)}

_{• Every other weekend} 

_{• Every other holiday}
