Prototypes S. Mark Taper Family Living Center is a 32-unit apartment complex that shares a campus with Prototypes Residential Substance Abuse Treatment Program and Prototypes Outpatient Program. The Housing Program at S. Mark Taper Family Living Center serves women and women with children with Substance Abuse and Mental Health Disorders. The Housing Program also services 64 scattered-site clients living around the Pomona area. The Housing Program provides case management services to help clients work toward acquiring permanent housing. Prototypes Housing Program is a fast-paced environment, as we strive to remain on the cutting edge of service provision. 

HealthRIGHT 360, a family of programs, is located across 13 different counties in California. HealthRIGHT 360 provides an array of medical and behavioral health services to some of the most vulnerable populations, ranging from primary care, mental health services, detox, substance use disorder treatment in both residential and outpatient settings, and transitional services for the criminal justice population.

Prototypes is a seven-acre campus with multiple programs serving clients in need of substance abuse and/or mental health treatment and/or housing. Prototypes Pomona Bridge Housing Program provides interim housing supportive services for people experiencing homelessness including linkage to recovery services, medication support, case management, crisis management, and transportation services/resources. 

On-Call Safety Navigator will be supporting clients enrolled in the interim programs funded by the Department of Health Services (DHS) and Substance Abuse Prevention and Control (SAPC) as well as clients residing in permanent housing contracts, as needed. On-Call Safety Navigator will provide supervision for afternoons, evenings, or graveyard shifts as scheduled by Housing Supervisor. 

Key Responsibilities

  • Conducts rounds during shifts to ensure units are clean and clients are safe.
  • Reinforces curfew and other program procedures.
  • Supervises clients and children during all assigned shifts.
  • Oversees clients’ independent living skills (cooking, cleaning, meal planning, shopping, etc.).
  • Provides follow-up to staff to ensure clients are moving toward goals as per service support plan.
  • Supports clients when dealing with interpersonal conflicts with others.
  • Conducts/participates in House Meeting to support clients and promote a safe and amicable environment in the units.
  • Encourages clients to be engaged in program and program demands.
  • Engages in conversation with Housing Supervisor for consultation if needed.
  • Monitors medication.
  • Creates, completes, reviews, and files SAM-L sheets.
  • Verifies, refills, reviews, and files Sign-In Sheets and Visitor Logs.
  • Ensures office is kept clean and organized and bathroom is properly stocked with supplies.
  • Controls the inventory for program (linens, food, office items, etc.).
  • Documents client records and uploads medical records in Welligent.

Education and Knowledge, Skills and Abilities

  • High school diploma or equivalent.
  • Two years’ experience working in a substance abuse treatment setting preferred.
  • Basic understanding of child development, child abuse, substance abuse, and self-sufficiency issues.
  • Experience working with substance abusers, chronically homeless, and/or reentry populations.
  • Experience with efficient and effective treatment and management of clients with co-occurring mental health and substance use disorders.
  • Knowledge of and experience providing trauma-informed care, housing first principles, and harm reduction services.
  • Computer literacy and proficiency in Microsoft Office.
  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.

Background Clearance:

  • Must not be on active parole or probation for at least two years.
  • Ability to obtain and maintain satisfactory background check.
  • Ability to obtain and maintain CDCR live scan clearance.
