Job Overview

The Licensed Vocational Nurse (LVN) is an integral part of the interdisciplinary team that supports clients in navigating evidence-based treatment plans and the healthcare system. They work in collaboration with Addiction Medicine, primary care, and behavioral health teams in the provision of Medication Assisted Therapy (MAT) and nursing support for other chronic diseases. They collect data and liaise with the Registered Nurse/Nurse Manager/Director of Addiction Medicine where necessary. They support treatment adherence via client education that assists clients in developing strategies for taking their medication regularly and effectively. Coordinates care for the client across the internal and external healthcare continuum. Utilizes the Nursing Process, Harm Reduction, and Trauma Informed Care modalities in all actions taken to advocate for and support the client in their planning and goals.

Key Responsibilities

Nursing Responsibilities: Within the scope of laws and regulations of California’s Board of Vocational Nursing, obtains data for assessments and care planning and collaborates with care team on carrying out nursing interventions. Works collaboratively with Health and Wellness and SUD staff. Participates in maintaining safety in the environment of care and contains and cleans biohazardous waste with appropriate PPE and supplies, when needed.

Organizational Responsibilities: Properly and consistently documents required information accurately electronic health records and in reports. Keeps up to date with operational and procedural requirements. Supports the interpretation of standards and processes to meet best practice standards established in the health services program. Provides direct care coordination support for clients to access all aspects of HR360 healthcare services.

  • Provides medication support, and participates with the registered nurse (RN) in client planning, especially as it relates to the teaching of interventions for client’s self-care.
  • Utilizes evidence-based tools for withdrawal management including, but not limited to, CIWA/COWS/PAWS.
  • Monitors and collects vital sign data, and enters into electronic health record.
  •  Pro-actively reaches out to RN and/or provider when clients experience a change in condition and implements orders on the unit.
  •  Remains willing and open to learn and engage in procedural changes necessary for a safe environment of care. 

Education and Knowledge, Skills and Abilities

Education, Certification, and Experience Required: 
  • Active nursing license.
  • BLS-First Aid Certification must be obtained within 30 days of hire.
  • Experience and interest in working with safety-net populations and in treating substance use disorders.
  • Experience working successfully with issues of mental health, criminal background, and other potential barriers to economic self-sufficiency.
Background Clearance:
  • Must not be on active parole or probation, clear with OIG database.
  • Ability to obtain and maintain satisfactory background check.
  • Ability to obtain and maintain CDCR live scan clearance.
