The Integrated Care Center Clinic is a federally qualified health centers that provide primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team based care in an integrated care setting. We are looking for a mission driven, service leader who works well in a team, has experience with managing and supporting staff, champions quality improvement, leads change, and is excited to innovate.

About the Behavioral Health Department:

 

The Behavioral Health Team is an amazing career opportunity for beginning and seasoned therapists to grow their practice experience in individual and group settings, as our patients span the full range of mental health disorders, from individuals coping with life stressors, singular traumas and mood disorders, to folks managing schizophrenia, personality disorders, addiction, and complex PTSD.

 

In addition to supportive clinical supervision and peer consultation within the team we offer a large training budget and numerous opportunities to learn from other departments at the Integrated Care Center.

Key Responsibilities

  • Responsible to balance a patient panel and schedule of other duties.
  • Provide shortterm, structured, evidencebased psychotherapy as part of an appropriate treatment plan and in accordance with HR360 contracts. 
  • Provide clinical therapeutic services to clients, including individual and group therapy and coordination of treatment for participants with cooccurring disorders. 
  • assessment, diagnosing, arranging, and implementation of client treatment plans. 
  • Provide brief intervention and crisis intervention as needed, including collaboration with other departments to provide behavioral health/mental health services and support. 
  • Maintain accurate and timely records of activities, progress notes, and services provided to each client. 
  • Prepare and utilize psychosocial educational materials as appropriate, needed or required. 
  • Use tests, measurements, and measurement scale for general and specific assessments to attain objective client criteria in the provision of services and evaluation procedures. 
  • Coordinate and collaborate with providers, case managers, and referral coordinators to improve health outcomes. 
  • Provide mental health screenings and initial assessments to clients referred through the Integrated Care Center (ICC) primary care clinic. 
  • Facilitate referrals to other services (e.g., case management, SUD treatment, specialty care and community resources) as needed. 
  • Work to meet client needs effectively and resolve individual barriers through followup, advocacy, and collaboration with HR360 staff and other community service providers. 
  • Participate in and/or facilitate team meetings. 
  • Other duties as assigned to support the mission of the organization. 

Job Requirements:   

  • Current possession of valid registration as an LCSW, LMFT, or Psychologist issued by the California Board of Behavioral Sciences; OR if ACSW/AMFT, BBS Registration to accrue clinical hours.

Background Clearance Required: 

  • Must not be on active parole or probation.

Education and Knowledge, Skills and Abilities

  • Psychosocial assessment.
  • Individual therapy/ counseling
  • Group therapy/ counseling
  • Shortterm, evidencebased counseling. 
  • Crisis intervention.

Knowledge and skills desired: 

  • Spanish-speaking preferred but not required. 
  • Culturally competent and able to work with a diverse population. 
  • Familiarity and comfort working with issues related to under-resourced populations including homelessness, substance use disorder, mental health disorder, and history of trauma. 
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
