If you are a qualified professional with a background in Hospice care and a passion for team leadership and process optimization, we encourage you to apply for our Admissions and Intake Manager position and become a key contributor to our impactful Hospice program at JSSA. This full-time role is an integral part of our hospice leadership team and will directly report to the Hospice Director. In this capacity, you will assume oversight of the Intake, Admission, and Discharge teams and their corresponding processes.
Responsibilities:
- Lead the end-to-end Admissions process, ensuring a seamless transition from initial referral to full admission
- Supervise Intake Coordinators, Admission Nurses, and Post-Discharge Nurses and Personnel
- Collaborate with weekend supervisor and weekend Intake Coordinators to provide continuity of care as relates to admissions
- Collaborate closely with Nurse Liaisons and Hospice Physicians to determine pre-eligibility of hospice referrals in a timely fashion
- Coordinate secure Live Discharges from hospice into community care settings in tandem with the Post-Discharge Nurse and other personnel
- Recruit, train, and onboard new staff members
Qualifications:
- Bachelor degree in Nursing (BSN) or equivalent years of nursing experience
- Active RN license to practice in the state of Maryland
- Minimum of 5 years of clinical Hospice or Home Care experience (Admission Nurse, field-based Hospice Nurse, Inpatient Unit Hospice Nurse, or Nurse Manager)
- Minimum of 3 years’ experience in managing and providing direction to nursing staff
- Exceptional verbal communication skills for effective interaction with the public, referral sources, and internal hospice staff
- High proficiency with Microsoft Office 365 and electronic medical records
- Strong organizational and technical aptitude
- Proficient in generating and analyzing productivity reports and other relevant metrics
