KHS reasonably expects to pay starting compensation for the full time position of Health and Wellness Lifestyle Coach I Bilingual I, in the range of $23.13 - $30.14 hourly.

Definition

 

Under direction of the Health & Wellness Programs Manager the Health & Wellness Lifestyle Coach performs a variety of educational and administrative support functions including clerical and data entry functions.  Assists with special projects as needed.

 

Distinguishing Characteristics

 

The Kern Health Systems (KHS) Health & Wellness Lifestyle Coach is a full time, hourly employee who is responsible for assisting the Manager, Supervisor, and department in a variety of clerical activities for a Knox-Keene licensed Health Maintenance Organization (HMO).

 

Essential Functions

 

  • Successfully complete certification as a Diabetes Paraprofessional Level I and/or an equivalent certification in a Nutrition program.

 

  • Demonstrate knowledge of various Health & Wellness program curriculum i.e., Diabetes Prevention Program, American Association of Diabetes Educators, American Heart Association, American Diabetes Association etc.

 

  • Demonstrate ability to provide both in-person and remote education on diabetes prevention, diabetes education, nutritional education, and other Health & Wellness program education to KHS members in a community setting.

 

  • Conduct member outreach and satisfaction surveys for Health & Wellness programs.

 

  • Demonstrate knowledge of community services and resources available to KHS members.

 

  • Assist Health Educators and Health & Wellness coordinator in class preparation, presentation, reminder calls, documentation, and data entry.

 

  • Call members to offer enrollment in the Health & Wellness lifestyle change programs.

 

  • Assist members with enrollment in Diabetes Clinics.

 

  • Assist members with appointment scheduling.

 

  • Mail Member correspondences, such as Welcome letters, Appointment confirmation letters, PCP Program enrollment letters and educational materials.

 

  • Composes simple letters, memos, confidential reports, and completes standard forms from information received in handwritten or typed form, generally without being given specific oral or written instruction.

 

  • Compiles information for logs; summarizes and types information retrieved from logs.

 

  • Creates manual and electronic files; searches for files; retrieves information from files; monitors use of confidential and miscellaneous files and their contents; purges files and/or information from files; closes files.

 

  • Sets up or revises filing systems; sorts and files a variety of printed and/or recorded materials.

 

  • Operates various commercial software systems, and specialized department systems to accomplish data input, manipulation, and output.

 

  • Responsible for weekly, monthly, and quarterly reports as assigned.

 

  • Assists in the development, organization, and coordination of internal and external meetings.

 

  • Reviews logs and updates computer system with departmental related information.

 

  • Proofreads departmental communications.

 

  • Maintains departmental inventory.

 

  • Acts as departmental liaison.

 

Other Functions

 

  • Assists management staff and other departments in completing special projects, as assigned.

 

  • Performs other job-related duties as required.

Employment Standards

 

Education

 

High School Diploma from an accredited school or equivalent; (diploma or GED certificate required). AADE Diabetes Paraprofessional Level 1 and/or equivalent Nutrition Certification required.  Diabetes Prevention Program (DPP) Lifestyle Coach Certification within 6 months of hire required.

 

Experience:

 

Two (2) years’ experience working in the managed care and /or health insurance industry.

 

  • Bilingual English/Spanish with certificate of linguistic proficiency (Tier I) required
  • Typing Certificate of 30 WPM required.

 

Other: Possession of valid California Driver’s License and proof of valid State required auto liability insurance. Required travel up to 15%.

 

Knowledge of:  Excellent verbal and written communication skills.    Knowledge of computerized on-line data entry systems, spreadsheets, and Microsoft Office Products or its equivalence.   Must be highly motivated, organized, and flexible, and possess good time management skills.  Ability to maintain high level of accuracy in data entry and report content.
