Description:

 

The Home Health Aid provides personal care, home management and other related home health supportive services in accordance with the DOH Matrix entitled, "Permissible and Non-Permissible Activities: Home Health Aide Services", in order to assist the individual to continue living in their home environment when there are disruptions due to illness, disability, social disadvantage or other problems in the home. The Home Health Aide is under the direct supervision of the licensed nurse.

 

Essential Duties and Responsibilities:

  • Assists with activities of daily living, such as bathing, dressing, grooming, toileting, and ambulation
  • Assist clients with ambulation and mobility around the house or outside (i.e., doctor’s appointments, errands, etc.)
  • Plans, prepares, and serves meals in accordance with prescribed diet, and encourages fluid intake
  • Acts quickly and responsibly in cases of emergency
  • Performs light housekeeping duties, such as laundry, vacuuming, dusting, and mopping
  • Assists with grocery shopping, picking up prescriptions, and attending doctor’s appointments
  • Provides companionship and engages in activities with the clients, such as reading, playing games, and going for walks
  • Measures intake and output of fluids
  • Monitors vital signs and reports changes to the physician, nurse, or agency supervisor
  • Keeps a detailed record of caregiving activities, including eating habits, mood changes, and physical activity levels
  • Collaborates with the client’s family members and other health care professionals to ensure the best possible outcome
  • Adheres to all safety precautions and infection control procedures
  • Maintains a clean and orderly environment
  • Observes appearance and gross behavioral changes in the client and reports to the agency supervisor.
  • Perform other duties as assigned

Knowledge, Skill Set & Qualifications Required

  1. Successful completion of a New York State Department of Health approved Home Health Aide training program as demonstrated by a valid Home Health Aide Certificate.
  2. High school or equivalent (preferred, not required)
  3. Sympathetic attitude toward the care of the sick, and ability to deal effectively with the demands of the job
  4. Ability to apply common sense understanding to carry out simple one or two step instructions.
  5. Ability to deal with standardized situations with only occasional or no variables.
  6. Good organization and communication skills and ability to read, write, understand, carry out directions, keep simple medical records, and function with minimal supervision.
  7. Ability to operate: walker, cane, crutches, wheelchair, commode, hospital bed, hoyer lift, household appliances (i.e., vacuum, refrigerator, stove, blender, toaster, etc.)
  8. Ability to portray exemplary customer service, speaking with respond, and avoid conflict.
  9. Authorized to work in the United States without sponsorship
