Job Summary
The primary responsibility of the Consultant Pharmacist is to provide support to the Value Based Care (VBC) team to ensure effective, safe, and cost-effective drug therapy in patients with kidney disease. The pharmacist has a key role in managing patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) by applying critical thinking, clinical knowledge, and evidence-based practices to promote quality improvement and care coordination. The pharmacist will target patients with high health care utilization for collaborative drug therapy management, as well as identifying gaps in care and optimize medication use to achieve best outcomes. The candidate should have a patient-first mindset with excellent problem solving and communication skills, who can thrive in an autonomous and evolving environment.
Responsibilities
- Participates in drug utilization review of medication profiles and documents all actions in a professional and consistent manner
- Performs medication therapy management via patient counseling and identification of medication related problems
- Consults with physicians and other healthcare providers of patients to address outstanding concerns and desired outcomes
- Provides patient counseling and answers a variety of clinical questions as presented by external partners, such as nurses, nurse practitioners, and physicians
- Participates in various special projects as requested by the department
- Maintains clinical knowledge of disease states relating to chronic kidney disease and end-stage kidney disease
Qualifications
- Must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree (required)
- Board certification (e.g. BCGP) or pharmacy residency (required)
- Registered pharmacist (RPh) licensure in home state current and in good standing (required)
- Registered pharmacist (RPh) licensure in multiple states (preferred)
- Medication Therapy Management (MTM) and/or disease state certifications [e.g. Certified Diabetes Care and Education Specialist (CDCES)] (preferred)
- Ability to obtain additional state licenses as required by the business
- Knowledge of EHR/EMR systems (e.g. Athena; Salesforce)
- Strong work ethic and ability to work as part of a high-functioning innovative team, while maintaining proper attention to detail and quality
Experience
- 2 years of clinical pharmacist experience (preferred)
- Expertise in medication therapy management, medication reconciliation, and in-depth medication reviews (required)
- Previous experience in providing pharmacy services and/or consultations for CKD and ESKD patients (preferred)
- Multiple state RPh licenses (preferred)
- Board certification (preferred)
- Working knowledge of Microsoft Office suite and Adobe products (required)
- Ability to speak additional language(s) (preferred)
Location
- Remote with very limited travel
This role has a salary range of $107,277 - $135,000 and regular, full-time employees working 30 or more hours per week are eligible for comprehensive benefits including Medical, Dental, Vision, Life, 401(K), Paid time off (PTO).
