Salary Range:       $38,000 - $40,000

 

Program Description:

Project Renewal’s Re-Entry Permanent Housing provides supported housing to 25 men/women with serious and persistent mental illness/dual diagnoses – who also have a history of significant criminal justice involvement/may be under parole supervision.  The program directly delivers and/or facilitates access to a broad range of services geared towards clients’ stabilization and community integration.

 

Overall Responsibilities:

Under the direction of the Senior Housing Counselor, the Housing Counselor is responsible for establishing and maintaining a constructive relationship with the clients, providing them with assistance to enable them to successfully perform everyday tasks, address their issues, stabilize their lives, and ultimately live independently in the community; performs related work as assigned by the Supervisor.

 

Essential Duties and Responsibilities:

The essential duties of the Housing Counselor include but are not limited to the following activities:

  • Provides clients with day-to-day assistance in handling their affairs and living in their apartments. Activities include assisting them with shopping, cleaning, cooking, doing laundry, making and going to appointments, providing escorts, delivering their medication weekly, and monitoring their taking of their medications.
  • On a weekly basis, participates in the distribution of medications to the clients, and on an ongoing basis, monitors the clients’ compliance with taking their medications. Documents client interactions weekly and works closely with the clinical team to assess and monitor client’s ability to manage their housing situation.
  • Monitors actions being taken on the clients’ 3-month housing plan.
  • Refers issues with any apartment to the Housing Coordinator for corrective action and or resolution with the Landlord.
  • Attends weekly staff meetings to discuss the status of each client with the Director and all other staff members.
  • Performs other duties as assigned by supervisory staff.

 

Qualifications:

  • High School Diploma or equivalent
  • A valid New York State Driver’s License preferred
  • Prior experience working with mental health consumers preferred

 

 

 

All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
