River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups. 

River Edge Behavioral Health is seeking a full-time Case Manager for our Assertive Community Treatment (ACT) program in Milledgeville, GA. In summary, the role involves organizing and implementing specific social services in the community, monitoring client progress, assisting with resource utilization, providing referrals, conducting skills training, maintaining contact with relevant agencies, preparing monthly case reviews, and offering transportation support for clients as needed, along with other assigned duties

Duties and Responsibilities

  • Implements and organizes the delivery of specific social services within the community.
  • Monitors clients' progress toward treatment goals
  • Assists clients in locating and utilizing community resources including legal, medical, financial assistance.
  • Provides referral services to those organizations as needed.
  • Provides resource linkage and skills training to clients living in personal care homes and transitional housing; assists in pre-release and release planning.
  • Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
  • Implements skills training for recovery management, activities of daily living, and social/interpersonal skills as needed.
  • Prepares monthly case reviews of progress toward treatment goals.
  •  Provide transportation for clients as needed.
  • Other assigned duties

Qualifications

  • Bachelor’s degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field.
  • Must have a valid GA Driver’s License.
  • At least 1-2 years of providing behavioral health service.
  • All applicants must pass a satisfactory background clearance and pre-employment drug test.
  • Provide 2 Professional references (Former employers only, no family members)
  • Demonstrate expertise and knowledge in mental health and substance abuse services and community resources.
  • Exemplify excellent communications skills, both verbal and written.
