River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups. 

River Edge Intensive Case Management Services is a community-based mental health team that provides wrap-around support services for individuals seeking recovery from mental illness who need more support than regular outpatient services. 

River Edge is actively seeking a full-time Case Manager for Intensive Case Management Services. The role involves delivering a comprehensive range of paraprofessional social services to patients, clients, and their families. The Case Manager actively participates in treatment team planning and conducts crisis intervention as needed. Duties also include assisting individuals in addressing personal and social challenges, providing supportive counseling to consumers and families, and serving as a liaison for social services. Additionally, the Case Manager undertakes case management responsibilities, coordinating services and resources, and conducts group skills training to enhance the overall well-being of individuals in the program.

Schedule: Monday- Friday 8:30AM-5PM

FLSA Classification: Hourly, Non-Exempt

Duties & Responsibilities:

  • Providing a broad range of paraprofessional social services to patients/clients/consumers and their families.
  • Participates in treatment team planning and performs crisis intervention.
  • Assists in dealing with personal and social problems.
  • Provides supportive counseling to consumers and families and/or serves as a liaison for social services.
  • Perform case management duties, and group skills training.

Qualifications

  • Bachelor’s degree in human services or a related field.
  • At least two (2) years of case management experience.
  • Provide 2 Professional references (Former employers only, no family members),
  • All applicants must pass a satisfactory background clearance and pre-employment drug test.

 

Competencies

Case Management: Ability to effectively manage and coordinate the various aspects of a client's case, including assessment, planning, implementation, monitoring, and evaluation of services.

Crisis Intervention: Proficiency in responding promptly and effectively to individuals experiencing acute emotional or psychological distress, employing techniques to stabilize the situation and provide immediate support.

Group Skills Training: Capability to design and facilitate group therapy or skills training sessions aimed at addressing specific social and emotional needs of clients, utilizing appropriate methodologies and resources.

Empathy: Capacity to understand and resonate with the emotions and experiences of clients and their families, fostering trust, rapport, and a supportive environment for effective intervention and counseling.

Communication: Strong interpersonal communication skills to effectively convey information, actively listen, provide feedback, and collaborate with multidisciplinary treatment teams, clients, and their families in a clear, compassionate, and culturally sensitive manner.
