Job Title:  Case Manager for Pathway of Hope Program (non-exempt)

Location:  Central Maryland Area Command (Baltimore)

Position Type:  part time, approximately 21 hours per week                         

Pay Rate:  $25.26/hour    

About this opportunity:

The Salvation Army, an internationally recognized non-profit, faith-based organization, has a job opening for a Case Manager, Pathway of Hope for the Service Center located in Baltimore, Maryland.  Qualified candidates will:

  • Conduct regular outreach to identified Corps or social services sites to identify eligible applicants.
  • Engage and build rapport with target population.
  • Conduct screening interviews with potential applicants in accordance with POH eligibility definitions.
  • Present potential participant to POH team to include the corps officer and Divisional POH Program Manager.
  • In coordination with Corps case workers/case managers, provide appropriate referrals for individuals not eligible for POH services, referring to Social Services and other resources as appropriate.
  • Perform intake that includes required documentation for admission, referral needs, explanation of program and expectations.
  • Develop a case plan with the participant based on the primary goals of housing stability, increased income/financial resources and skills, and self-determination which includes short-term and long-term goals with objectives.  Update case plan as needed.
  • Schedule regular meeting times with participants to develop and review goals and objectives.  Conduct home visits as determined by the case plan.
  • Provide information and referral services.
  • Additional responsibilities as assigned.

What we are looking for in you:

Bachelor’s degree in Social Work, Human Services or related field; 

 

AND

Three years of experience performing Social Work, preference given to experience utilizing a strengths base approach to case management

 

OR

 

Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

 

Licenses and Certifications:

 

Valid Driver’s License and must meet TSA MVR Requirements.

 

Physical Requirements and Working Conditions

  • Ability to meet attendance requirements.
  • Ability to read, write, and communicate the English language.
  • Ability to learn and concentrate.
  • Duties are performed seated or intermittently sitting, standing or stooping
  • Occasional lifting of light objects (less than 25 lbs) and occasional lifting of objects 25 to 50 pounds may be necessary.
